This subchapter applies to a group health benefit plan that is delivered or issued for delivery in this state and that is: (1) an accident and health insurance group policy; (2) a group policy issued by a group hospital service corporation operating under Chapter 842 ; or (3) a group health care plan provided by a health maintenance organization operating under Chapter 843 .
