"Individual contract"
Ind. Code § 27-13-1-21
in effectSec. 21. (a) "Individual contract" means a contract for health care services that:
(1) is issued to; and
(2) covers;
an individual.
(b) An individual contract may include coverage for a dependent of the subscriber.
