3-56-030 Application. Applications for a wheel tax license shall be filed with the city clerk, on a form provided by the city clerk, and shall contain the following information: (1) the name and address of the applicant, (2) a description of the vehicle for which the wheel tax license is desired, and (3) any other information that the city clerk may reasonably require.
