2-112-290 Records and forms.

It shall be the duty of the Department to provide the necessary books for keeping a record of all transactions of the Department, and such statistical information necessary for the efficient working of the Department. The Department shall also keep on hand all necessary forms for use by physicians and midwives and shall furnish them with such forms upon application. (Prior code § 9-21; Amend Coun.
