7.04.080 - Animal vaccination certificate. "Animal vaccination certificate" is the certificate approved in form by the Health Officer which is issued and signed by the veterinarian who inoculates an animal for rabies stating thereon the name and description of the animal, the date of such vaccination and the type of vaccine used by the veterinarian.
