Oklahoma City Municipal Code § 38-168
in effect§ 38-168. - Forms and format.
(a) Forms for permits required under this article shall be printed and shall contain ample and appropriate space for the identification of the person receiving same, the specific activity or combination of activities for which the permit is issued, the date of expiration of the permit and a pledge in substantially the following language:
"In consideration of the issuance of this permit to me, I hereby faithfully agree to observe all rules of safety, sanitation and sportsmanship, together with all provisions of City ordinances, State Game and Fish laws, and rules and regulations of the Fish and Game Commission of the City, while upon the lands or waters included in all City-operated reservations, and to hold the City, and any person acting for or on the behalf of the City, harmless for any injury to life, limb or property incident to the exercise of the privileges hereby granted."
(b) Such permit forms shall be supplied to the Chief Ranger or his assistants on the premises and also to the City Treasurer and other persons who may be authorized by the City Council to issue and sell, upon application, permits for the privileges of this article.
