§ 6.80.040 — License—Application

6.80.040 - License—Application.

The application for the license that is required by Subsection (A) of LVMC Section 6.80.030 shall be made on forms that are suitable to the Director and filed with the Department. The application shall set forth the following information: (A) The name and address of the applicant; (B) The date and time when the dance will be conducted, if the application is for an individual dance; (C) A description of the location to be licensed, including without limitation the dimensions of the interior of the location, the dimensions of the dance floor, the seating arrangements, the location of the dance floor within the location and the dimensions of the parking area; (D) The provisions that have been made by the applicant for security and the names and addresses of the persons who will supervise the license location; (E) The manner in which admission will be permitted, and, if admission will be by a set admission charge or the donation of money, the amount of the charge or the donation that will be required; and (F) Such other information as the Director may reasonably require with respect to the security and conduct of the dance, dancehall or nightclub.
