The name, address and telephone number of the owner of the premises to be served, if same is not the accountholder, and the telephone number and address of the owner, (5) the address to which the accountholder desires the bills be sent and the relationship of the billing address to the accountholder's, if different from the service address, (6) if unmarried, the telephone number of a relative or friend to be contacted in case of emergency, (7) a statement by the requesting or executing party as to relationship to any company, corporation or partnership in whose name or for whom service is being established, where applicable. (b) In addition, upon request each accountholder must provide and keep current and updated the following information: (1) changes in the accountholder's name or marital status, (2) any aliases or maiden names used or previously used by accountholder and accountholder's spouse, if any, (3) accountholder's status such as the contracting customer or representative of the contracting customer, for example: individual, or married person, or owner of a business, or officer or agent of a business entity, or officer or agent of a government entity, or court appointed trustee, receiver or administrator, and (4) any occupants or co-occupants residing, living, or receiving or benefiting utility services at the service address, hereinafter referred to in this section as "occupants." (c) All accountholders and utility customers may be required to enter into a written contract for utility services. Provided, however, the Director, in his or her discretion, may permit the initiation of utility services within or without the corporate limits of the City without a signed written contract, in which case this Code shall be deemed to be the written documentation of the contract for utility services with the accountholder and the accountholder's request for service shall be deemed the accountholder's consent and approval of the terms of service in this Code, and all amendments and additions thereto. Unless earlier terminated by the City or by the accountholder in accordance with Utility Customer Service Division policies and procedures, the contract for utility services will continue from month to month and the accountholder will be responsible for all charges to the service address. The accountholder and the accountholder's spouse, if any, shall be jointly and severally liable for all debts and obligations under the contract for utility services requested or received by either. Upon the provision or availability of utility services to the service address the accountholder and all utility customers shall be subject to all provisions, requirements and conditions set forth in this Code and the contract for utility services. The accountholder and all utility customers shall be responsible for water use and utility services at or to the service address. (d) The application shall constitute a general waiver on the part of the applicant and accountholder of any claim for offset, damage or breach against the City resulting from or occasioned by any defective plumbing or appliance or installation installed or caused to be installed by the owner, lessee or occupant at the service address, or resulting from or occasioned by any initiation, interruption or failure of the supply of water or by discontinuance with or without notice of the water supply service at any time. (e) The application for water service shall constitute and be construed as a continuing offer on the part of the applicant and accountholder to purchase utility services from the City delivered by the Utility Customer Services Division thereof as set forth in this Code. (f) All accountholders to receive service and all customers receiving any utility service from Oklahoma City agree as a condition and requirement of such service to read and conform to this Code.
