Oklahoma City Municipal Code § 18-38
in effect§ 18-38. - Form of application.
Prior to any electrical installation, alterations, or additions to existing wiring, electrical fixtures or appliances, except as specifically exempt in this Code, an authorized contractor shall apply for a permit with the Development Services Department either in person, or if he has qualified for contractor's option, by telephone, electronically or fax, and provide the following information: street address of the premises where electrical work is to be done; building permit number; name of electrical contractor; kind of building; etc.; list of work to be done; and any other information specified by the Director of Development Services or designated representative.
