6.80A.040 - Application—Contents. Any person who is required to have a permit under this Chapter must file an application with the Director. The application must be made upon a form provided by the Department and shall include the following information: (A) The dates on which the applicant desires to conduct business; (B) The location(s) from which the applicant desires to conduct business; (C) Location agreement(s) or contract(s); (D) Designated agent(s) for additional locations; (E) A description of the selling methods to be used and the nature of the products or services to be offered; and (F) Such other information or documentation as the Department may require in order to establish the applicant's suitability and fitness for approval.
