Permit fees for individuals or enterprises that transport hazardous materials shall be as follows:

3—5 ..... | 100.00
6—10 ..... | 200.00
11—50 ..... | 300.00
51—100 ..... | 400.00
101—500 ..... | 500.00
More than 500 ..... | 600.00
