§ 13-37. - Application required; oath; contents. A written application for a permit required by the provisions of this article shall be subscribed and sworn to by the president or other chief officer of the organization making the application and filed with the Supervisor of Licenses 15 days prior to the time when a permit is desired. The application shall contain the information required by 18 O.S. § 552.3, as amended, and may be a duplicate of the information provided to the Oklahoma Tax Commission.
