§ 7-262 APPLICATION FOR PERMIT. The application for a permit to operate an overweight and/or oversize load or loads on the streets of the city shall contain: (a) The weight, kind and complete description of the equipment to be operated; (b) The kind of commodity to be transported and the certification of the commodity’s weight; (c) The name of the street or streets over which the equipment is to be operated, the date or dates of such operation, the time of the operation and the number of trips to be made; and (d) The date and signature of the applicant.
