Sec. 12.14.130. - Same—Form.

The notice of infraction must be on the form which is prescribed by the director of maintenance and engineering and must contain the following information: (1) The location at which the violation occurred; (2) The date and time of the violation; (3) The section of the code which allegedly is being violated; (4) Information which advises of the manner in which, and the time within which, the notice of infraction should be answered; (5) Any other reasonable information which is prescribed by the director of maintenance and engineering; and (6) The signature of the person who issues the notice of infraction.
