4.12.070 - Negligence-based claims. Claims for damages against the City which are based on the alleged negligence of the City, its agents or employees, shall be presented to the City Clerk in writing and shall set forth the time and place of the accident, names and addresses of all witnesses, a statement of the facts surrounding the accident, a description of the injuries suffered and the amount of damages claimed.
