Las Vegas Municipal Code § 11.58.080
in effect11.58.080 - Structure permits—Application.
The application for a special permit provided for in Section 11.58.060 shall:
(A) Specifically describe the vehicle, combination of vehicles or special mobile equipment, and load thereon, including the width, length, total gross weight with load imposed and number of axles of the vehicle or equipment and the width, length and height of the building or other structure to be moved thereby;
(B) Indicate the particular streets, highways or alleys over which the special permit to operate or move is requested;
(C) State the date and time for which such special permit is requested.
